Q:  What is Coltrane LIFE Center?

A:
 Established in 1974, the Coltrane LIFE (Living Interest for the Elderly) Center, the only adult day health services program in Cabarrus County, helps older and disabled adults continue to live at home rather than be placed in a nursing home.  Serving Concord, Kannapolis, and surrounding cities and counties, the Center, a private, nonprofit organization, provides quality daytime health care services and meaningful recreational and socialization activities in a home-like setting Monday through Friday, as well as transportation, nutritious meals, beauty/barber shop services, and assistance with personal care needs.  These services enable family caregivers to work, run errands, relax, or tend to other matters during the day without worrying about the safety and care of their loved ones.

Adults from Cabarrus and surrounding counties, whether they live alone or with family members. While at the Center, the participants are safe, supervised, interacting with others, participating in meaningful activities, and receiving health care services.

Volunteer:
We love our volunteers! You are welcome to volunteer your time and services by working with participants one-on-one, helping with field trips or special programs, assisting with office work, and more.  See application form on this web site under “Giving.”

Wish List Donations:
See our monthly LIFEline newsletter (under "Newsletter" on this website) for a current Wish List for the Coltrane LIFE Center.  The donation of items on this list greatly enhances our ability to serve our participants.

Monetary Donations:
Financial contributions of any size are very much appreciated.  You may also give in honor or memory of a loved one. See donation form on this web site under “Giving.”


Q: How do I enroll a loved one in Coltrane LIFE Center?

A: To enroll a loved one, first stop by the Center between 9 a.m. and 5 p.m., Monday-Friday, for a tour of the Center, or call or e-mail Carrie Holt, Family Services Coordinator, at 704-788-1215 or cholt@coltranelifecenter.org to make an appointment for a tour.

Payment for the Center’s services is flexible and affordable. The Center collaborates with federal, state, and county agencies on behalf of participants who need financial assistance. Through generous support from United Way and many others from the community, the Center maintains affordable rates and offers some scholarships.

Before attending on the first day, the family must complete and return the Application for Enrollment, and the physician must complete and return the Medical Examination Report along with the results from a TB skin test or recent chest x-ray.
