Enrollment

To enroll a loved one, first stop by the Center between 9 a.m. and 5 p.m., Monday-Friday, for a tour of the Center, or call or e-mail Carrie Holt, Family Services Coordinator, at 704-788-1215 or cholt@coltranelifecenter.org  to make an appointment for a tour.

Payment for the Center’s services is flexible and affordable. The Center collaborates with federal, state, and county agencies on behalf of participants who need financial assistance. Through generous support from United Way and many others from the community, the Center maintains affordable rates and offers some scholarships.

Before attending on the first day, the family must complete and return the Application for Enrollment, and the physician must complete and return the Medical Examination Report along with the results from a TB skin test or recent chest x-ray.
